Rehabilitation Therapy Symposium:  SNF Rehabilitation Therapy Best Practices in a Value-Based World

For the 9th year, the American Health Care Association and ADVION are partnering to co-host the 2024 SNF Rehab Symposium.  The number of beneficiaries choosing Medicare Advantage is on the rise and nursing facilities are faced with meeting requirements of various MA plans, ACOs, and I-SNPs among other models. CMS has also increased the number of measures in the SNF Value Based Payment Program (VPB) and intends to have all beneficiaries under accountable care payment models by 2030. This half-day event offers clinical and practical insights, and strategies to help attendees advance the delivery of quality care, treatment, and outcomes. The presentations will focus on cutting-edge innovative approaches related to rehabilitation services successfully navigate the increase in value-based models in long term care. Presentations are designed to address the information and educational needs of nursing facility staff including administrators, directors of nursing, directors of rehab, nursing facility owners and operators as well as leaders of rehab therapy providers.

Skilled nursing facilities (SNFs) face myriad challenges as they care for their residents and patients. Despite the desire for innovation, the adoption of new technologies has been slow. This session will describe Signature Rehab’s journey to introduce a new, smartphone based remote monitoring platform to drive collaboration between rehabilitation and nursing teams. This presentation will illustrate the roadmap followed by Signature Rehab, detailing the design and implementation of technology-driven workflows and their significant impact on critical areas such as falls reduction, interdisciplinary collaboration, patient and provider engagement, and documentation to support care justification.

ISNP and the Importance of Resident Centered Care in a Managed Care World

Managed Medicare (MA) continues to grow. The institutional special needs plan (ISNP) is one type of MA plan available to eligible residents of skilled nursing facilities (SNF). For this to be an option, the SNF must choose to participate in an ISNP. This value-based approach allows SNF providers the opportunity to positively impact revenue. Upside is realized when quality metrics are met resulting in a decrease in overall spend.  This session is designed to provide you with strategies we have found to be successful in the development, implementation, and updating of a comprehensive ISNP model of care.

The current managed care delivery model is broken resulting in waste, limited patient outcomes, inefficiencies and costs that lead to low profits, low patient satisfaction and bad business. Conventional care models are under enormous cost pressure, the patient population is aging, and we have more underserved and rural populations with unmanaged acute and chronic conditions. This presentation focuses on using hybrid care models consisting of efficient SNF and Outpatient Therapy with faster transition to care at home using Wearables, Digital Health, Telehealth and AI to help patients, providers and payers survive and thrive in Managed and Value Based Care environments.
