This session will describe Signature Rehab’s journey to introduce a new, smartphone based remote monitoring platform to drive collaboration between rehabilitation and nursing teams. This presentation will illustrate the roadmap followed by Signature Rehab, detailing the design and implementation of technology-driven workflows and their significant impact on critical areas such as falls reduction, interdisciplinary collaboration, patient and provider engagement, and documentation to support care justification. |

Managed Medicare (MA) continues to grow. The institutional special needs plan (ISNP) is one type of MA plan available to eligible residents of skilled nursing facilities (SNF). For this to be an option, the SNF must choose to participate in an ISNP. This value-based approach allows SNF providers the opportunity to positively impact revenue. Upside is realized when quality metrics are met resulting in a decrease in overall spend.
